From 1922, the Annual Return and Record (B.F.1.A) format changed to having a front cover of a folded form which gave details of Division address, member in charge, Divisional Surgeon (if one), numbers in the Division, practices held, cases attended, hospital attendances, parade and examination details, and how the Division financially supported. The form recorded the Ambulance Cadet District number, date of enrolment, name, rank, ambulance qualifications, number of drills, public duties and transport duties attended, presence at the annual inspection and date, and whether the member passed the annual re-examination. There are sometimes handwritten modifications to the B.F.1.A to give the Ambulance Cadets' age and details of which section they belonged to (senior or junior).

From 1932, the Annual Return and Record (CADETS C.F.1.A) was introduced, and after 1935, it was referred to as CADETS B.F.C.1.A. This form recorded the Ambulance Cadet District number, date of enrolment, name, date of birth, rank, ambulance qualifications, number of drills and duties attended, presence at the annual inspection and date and whether the member passed the annual re-examination. Also recorded were the number of service stripes or badges, and number of certificates or special badges held. By 1940, a column in the form was added to record if senior Cadets had an A.R.P certificate, and by 1943, the number of Civil Defence Duties attended. By 1948 the Annual Return and Record (B.F.C.1.A.) also recorded on the front-page numbers of Cadets over and under the age of 14, and any junior Cadets aged 8-11 years. Inside the form the columns referring to A.R.P. Certificates and Number of Civil Defence Duties have been removed.

The forms also contain space for additional remarks on members such as resignations, transfers, missed inspections, evacuations.
