The forms are largely the same Annual Return and Record forms used by Divisions of the St John Ambulance Brigade, which, on the front cover, largely provide the opportunity to record administrative information relating to the District, including membership, number of instructional meetings and duties held, number of cases treated, number if ambulances owned, number of medical depots, number of beach huts, number of road huts, and number of road boxes which are utilised by the Division. These sections are not completed.
The inside pages contain spaces for recording the date of enrolment in the Brigade, names of the members, the members' rank, qualifications, number of instructional meetings attended, inspections attended, annual re-examinations undertaken, transport, hospital and nursing duties completed, public duties completed, transport duties, hospital hours, nursing aid hours, and whether the members are enrolled in the National Hospital Service Reserve, or Voluntary Aid Detachments. Of these, the sections that have been completed for the District Staff is the members' name, rank, and date of first aid qualification.
