The forms largely contain administrative information relating to the headquarters of the Division, details of Division address, member in charge, Divisional surgeon (if one), numbers in the Division, practices held, cases attended, hospital attendances, parade and examination details and how Division financially supported, number enrolled in VAD. The main form added a record of the members District number and number of hospital attendances, Air Raid Precaution instructors, rank and file certificates and reservists, membership of the Civil Nursing Reserve, completed Anti-gas qualifications and ARP attendances in hospital, first aid post or mobile unit. The BF1N is modified in 1944, the APR columns removed, and Civil Defence Attendances added. It is changed again in 1945, and the Civil Defence Attendances no longer recorded.

They also identifies the numerical strength of the Division largely including Divisional Surgeon, Lady Divisional Superintendent, Nursing Officers, Lady Ambulance Officers Ambulance Sisters and statistics on the work of the Division, date of enrolment, name, rank, ambulance qualifications, uniform status, number of practices and public duties, inspection, whether passed re-examination, services medals, if belonging to a Brigade Voluntary Aid Detachment, and general remarks. Remarks included recording if the member was working in hospital, working in France, on the land, moved from area, public, transport, hospital, child welfare, nursery, and blood transfusion duties.

The forms also contain space for additional remarks on members such as resignations, transfers, missed inspections, work in military and civilian hospitals, illness, air raid precaution, evacuations.
