The forms largely contain administrative information relating to the headquarters of the Division, names and qualification of the Divisional Surgeon, rank and name of the Officer or member in charge; the numerical strength of the Division largely including Divisional Surgeon, Lady Divisional Superintendent, Nursing Officers, Lady Ambulance Officers Ambulance Sisters and statistics on the work of the Division.
In 1906 the BF2 Brigade Record Sheet included enrolment number and date, name, rank, uniform use, number of drills, duty parades, inspection, pass of annual re-examination, whether efficient and general remarks. In 1912 the BF2 added a record of service badges and asked if the member belonged to a Brigade Special Reserve.
