Statement of Service forms (B.F.4.) created to collect and contain information on members of the St John Ambulance Brigade who are applying for a Service Medal or Bar, transferring from one Division to another, or for their personal record. The forms contain administrative information, including the member names, date of which they undertook their first aid re-examination, drills, inspection, public duties, and when they received their Service Chevrons. There is also a space for additional remarks to be made.
