The forms contain administrative information relating to the headquarters of the Division, names and qualification of the Divisional Surgeon, rank and name of the Officer or member in charge; the numerical strength of the Division largely including Divisional Surgeon, Lady Divisional Superintendent, Nursing Officers, Lady Ambulance Officers Ambulance Sisters and statistics on the work of the Division. Please note these records are categorized using St John Ambulance Brigade form numbers (e.g. BF1N).

The form also contains member specific information including membership number, date of enrolment, surname and initials, rank, qualifications, uniform, meeting, duty hospital attendance, motor ambulance attendance, inspection presence, passed examinations and awards.

During the Second World War, the forms began to include war service specific information on the enrolment of members in V.A.D., the Civil Nursing Reserve and in Air Raid Precaution training and attendance, Child Welfare Clinics and Nurseries. The form also provides information on the number of members enrolled in the Voluntary Aid Detachments (V.A.D.), Air Raid Precaution instructions, rank and file certificates, and medical comfort depots maintained.

The form also contains a space for additional remarks on members such as resignations, transfers, missed inspections, air raid precaution, evacuations. There are also instances of honorary members contained in some items.
Please note the file contains some additional and supporting papers including a form relating to the postings of a V.A.D, 1940; several re-examination forms (B.F. 11), 1941; member-related correspondence, 1946-1947.
