A round, gold medal. The obverse depicts the effigy of Her Late Majesty Queen Victoria derived from a bust carved by her daughter, The Princess Louise, Duchess of Argyll with a circumscription. The reverse depicts, between sprigs of St John’s Wort, five circles showing, the Imperial Crown, the Royal Arms, the Arms of The Prince of Wales who was the first Grand Prior under the Royal Charter of 1888, the Crest of that Prince of Wales and the Arms of the Order as they were immediately prior to 1926, with a circumscription.
The ribbon has three black and two white stripes of equal width. The central black stripe is dissected by a 3mm gold stripe.
The medal is suspended from a straight bar fixed suspension on an ornate foot fitting positioned at 12 0'clock. It is worn with the bright plated steel brooch with soldered joint and catch on the top reverse of the ribbon. - Production place

Ultra Long Service Medal of the Order of St John
In 2020, the Ultra Long Service Medal was introduced to recognise those giving more than 50 years of service to St John Ambulance and the Order of St John. This medal is the proof copy that was sent to Queen Elizabeth II and the Grand Prior for inspection.
